Modern nuclear weapons are designed to explode above a target rather than by directly contacting the ground. Fallout only becomes dangerous when the radioactive material mixes with the dust and dirt and debris and scatters across a large distance. This isn't typically the case with the fireball far above the target, destroying the target by the force of the explosion but not spreading any of the debris for hundreds of miles. Sure, radiation would be a problem but it would not realistically result in the events shown in Threads.

The reason why they named the original film "Threads" is now more pertinent than ever: You have all these global supply chains that make everybody reliant on everybody else to maintain high tech society. A temporary disruption of these supply chains already causes economic crises. Now imagine what a total collapse of global trade would lead to. Pockets of humanity would undoubtedly survive, but it's questionable whether they could recreate the high tech society we have now, because the first industrialization used up all the easy to reach fossil fuels which they'd need for the second round. The survivors might be stuck on Amish levels of tech in perpetuity for lack of energy sources.

The real thing the film exaggerates is nuclear winter and radiation. Even with the much larger arsenals at the time the film came out the nuclear winter predictions were ridiculously overexaggerated, not to mention the complete pseudoscience about the ozone layer disappearing and everyone being irradiated ten years in the future which is total bullshit; In the present day with the much smaller arsenals not only would you see a significantly smaller initial death toll but you'd also see virtually no radiation threat since it's mostly thermonuclear airbursts nowadays. The film's main success imo is showing how the greatest threat after nuclear war is ultimately the breakdown of government and trade leading to mass anarchy and starvation, which is entirely accurate to real life imo.
