I turned it off when he was killing that woman and her kid in the field from the hunting stand. I thought this film would be amazing from the trailer, but I got no joy or satisfaction from the movie as I watched it. Perhaps that serves me right.

It was kino. I still think about it, the hell tour was legitimately great.
Any scene that was pretentious was obviously meant to be viewed as a reflection of Jack's own pretentious nature, like him standing in an alley with the cards and his own Mr. Sophistication moniker.

The basic idea is if you do bad shit without any form of repentance or regret you go to hell. That's the basic message of the movie.
The depiction of hell is excellent, drawn from Dante's Inferno. In fact the whole movie is Vergil talking with Jack and growing increasingly disgusted by not only Jack's actions, but Jack's fondness of them.
For me the most memorable moments were the least disturbing, though there's certainly some extremely dark imagery.
To answer your question you'd get an entertaining rumination of a pretty common-sense theme.

I always wondered, that if Jack truly regretted what he'd done, and Vergil listened to that regret and desire to repent, would Vergil have lead Jack to Purgatory instead? Would his opinion have been markedly different from the revulsion and pity he felt instead?

Yeah I don't doubt it's a good version of a serial-killer movie, I just don't have an interest in watching a bunch of people get horrifically murdered for no good reason regardless of what the final message is

I'm not judging, when I was 17 or 18 I probably would've watched this and felt like I got something out of it, I just lost my taste for stuff like this

Part of what I liked about it is it doesn't really get into the killings as much as I would've thought. It's a lot less edgy than something like terrifier. All of the killings serve the plot, and mostly you only see the aftermath of those killings, often in very brief moments.
The sadism in the film is portrayed not only negatively, but also as somewhat cringeworthy and pathetic.
As for nihilism, it's about as far from nihilistic as it gets. I'd even call it a movie that atheists could never understand.

It's a great film, definitely recommend it if you can handle the subject matter. I really only have two complaints with it.

That scene where the camera is spinning in a circle for like two minutes was extremely obnoxious and I had to look away because it was giving me a headache. Not sure why it went on for so long.

That part where Verge or Jack mentions "real art" or something and a montage of Von Trier's OWN films starts playing. I actually said "Oh fuck off" out loud when I saw that. Needless, completely irrelevant masturbation in a film that has no reason to be there.
